The inspector’s account

04/23/2026, Licensing Program Analyst (LPA) Loera arrived unannounced to conduct a Case Management - Incident visit and met with Program Manager, Terry Roach. The purpose of the visit was to follow up on self reported incident that was submitted to Community Care Licensing (CCL).

CCL received an incident report on 03/27/2026, stating on Friday, 03/27/2026, at approximately 1:28 pm, Program Coordinator was checking the daily medications disbursements and noticed that client (C1) had not been given their lunch time medication. C1 was scheduled to take two pills with their lunch.

Per conversation with Program Manager, staff scheduled to dispense medication forgot to give C1 their scheduled medication.

Documentation for Pace Solano instructs C1 to take two tablets by mouth with food at 12pm during program hours.

Facility came up with a preventative plan to ensure medications are given as prescribed.

Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.
