Facility Evaluation Report
On 05/22/2025 Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr inspection for this facility and met with Administrator, Terri Roach. The facility currently provides care for 54 clients, 43 of which were at the facility at the time of visit. 19 staff were present during inspection.
LPA conducted a tour of the facility with Administrator; facility was found to be clean and at a comfortable temperature with all exits free from obstruction. Fire extinguishers were found to be last charged 09/2024. Facility gets fire alarm system inspected and was last tested on 04/29/2025. Emergency disaster drill was last conducted on 01/30/2025. Toxins and cleaning supplies are stored in a designated cleaning supply closet inaccessible to clients. Sharps are stored in a locked cabinet located in the kitchen. LPA was informed clients bring their own lunches to day program with additional snacks being provided. Facility has multiple activity rooms, each equipped with sufficient amount of supplies.
LPA reviewed client P&I monies and conducted spot medication count. LPA conducted a file review of client records and found all items to be in order. In addition, LPA conducted a file review of staff records and found staff to have the required amount of annual training and current CPR & 1st Aid.
