Facility Evaluation Report
At approximately 11:50 AM, Licensing Program Analyst (LPA) Magdaleno arrived unannounced to conduct a required 1-year annual inspection and met with Program Director (PD) DeMarcos Ayers. Facility is an Adult Residential Facility (ARF) with forty four (44) clients in care. LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation.
At approximately 12:45 PM, LPA initiated a tour of the facility with Program Director and observed the following: Facility consists of one (1) story, was a comfortable temperature, and passageways were free from obstructions. Licensed facility Our House shares a campus with a locked Psychiatric Health Facility (PHF) not licensed by Community Care Licensing. Facility's fire extinguishers were observed charged and last serviced 12/25. Smoke and Carbon Monoxide detectors are hardwired and were tested by a third party vendor on 1/26. Water temperatures in a spot check of seven (7) client accessible sinks, including bathrooms and shared areas, read between 110.4- and 119.3 degrees F which is within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens, hygiene, and paper products available for clients. Facility laundry room was observed locked, per PD clients are responsible for doing their own laundry and the laundry room will be unlocked when required for their usage. Facility maintains industrial laundry machines to wash client towels/bedding on a daily basis.
Cabinets containing cleaning supplies and other items that could pose a risk were locked. Facility has at least two (2) days of perishable food and seven (7) days of non-perishable foods. Facility kitchen was observed clean with food properly stored and labeled. Facility kitchen provides food for both the licensed facility and PHF. Facility has a computer room with internet accessible computers for client usage. There is an outdoor recreation area with exercise equipment, games, shaded seating areas, and a smoking patio. PD informed LPA that there are staff led activities conducted outside which include a wiffle ball home run derby and basketball matches.
