Facility Evaluation Report
Licensing Program Analyst (LPA) Magdaleno arrived unannounced to conduct a case management-other visit and met with Administrator Nicole Paiste and Program Director DeMarcos Ayers.
LPA is following up on a self-reported incident report (IR) received by CCL on 11/26/2025. Per IR, client (C1) took both AM and PM doses of prescribed medication. C1 LIC602-Physicians Report indicates that C1 is able to manage their own medications. Interview with Program Director indicated that C1 will receive higher levels of support while they learn their medication routine. Staff have also received further training on providing support to C1. LPA is requesting facility submit in-service training logs for staff.
LPA is also following up on a request from a third-party to ensure food levels in the facility remain in compliance. LPA toured the kitchen and observed food to be stored and labeled in a safe manner and to be within compliance in quantity. Facility serves meals three times a day and ensures bowls of fruit and snacks are available around the clock for clients.
No deficiencies cited. Exit interview conducted with Program Director, whose signature on form confirms receipt.
