At approximately 10:20 AM Licensing Program Analyst (LPA) Elias Magdaleno arrived unannounced to conduct a case management - other visit and met with Program Director (PD) DeMarco Ayers.
The visit reason was to discuss management structure at the facility as well as to discuss incident reports submitted 9/18/2025. PD explained management structure and reporting procedures.
No deficiencies cited. Exit interview conducted with Administrator, whose signature on form confirms receipt.
