Facility Evaluation Report
Licensing Program Analyst (LPA) Alviso aarrived unannounced to conduct an Annual Required - 1 Year inspection and met with Licensee/Administrator Jean Stephens.
LPA observed a caregiver, Robert, on duty during the inspection. Currently there are two (2) clients in care, both were attending day program.
Facility has an approved fire clearance for 6 residents, of which two (2) may be non-ambulatory. Facility has smoke alarms, and carbon monoxide detector (s), all were working properly; Fire Department did clear the facility fire clearance annual inspection on 1/17/25. Licensee stated the fire department does an annual check on the fire clearance of the home.
LPA reviewed two (2) resident files. Files were complete.
LPA reviewed four (4) staff files. All staff have required criminal record clearance. LPA reviewed staff training. All staff have CPR certification and first aid certification.
LPA toured the facility with the Administrator. Hot water was measured at 112. degrees Fahrenheit, which is within regulation. All exits were free and clear of obstructions. The home was at a comfortable temperature during the inspection. Facility had sufficient food supply.
Facility had sufficient linens, hygiene products, cleaners/disinfectants, and paper products. All medications were centrally stored and inaccessible Disinfectants/cleaners were locked up and inaccessible to clients in care.
