Complaint Investigation Report

Review of R1 chart notes indicated visitation is being logged. Further review of R1 files indicated that R1 does have a Durable Power of Attorney, however, there is not a court order against visitation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated .

No deficiencies cited. Exit interview conducted with Administrator, whose signature on form confirms receipt.
