Facility Evaluation Report

At approximately 9:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to conduct a case management visit in regards to the closure of this facility. The Department received a letter on 09/15/2025, signed by the Licensee, expressing their desire to close the facility. The letter explained all residents had been moved from the facility and the Licensee had lost control of the property. The letter stated they wished to surrender their License and close the facility. LPA requested additional information via email. During LPA's inspection of the facility, LPA observed the facility appeared to be vacant. The windows accessible from the street had their blinds closed preventing visual access to the inside. LPA knocked and rang the door bell several times receiving no response. LPA telephoned the property owner to request access to the building, but received no answer.

Upon receipt of the requested information from the Licensee, LPA will email a copy of this report to the Licensee for signature.

The closure date of this facility will be determined upon receipt of the requested information.
