Facility Evaluation Report

At approximately 11:30AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to conduct a case management visit for the purpose of amending a report. On 07/29/2025, LPA conducted a Plan of Correction visit where LPA issued a civil penalty to the Licensee for not correcting the violation. After the penalty was issued, the Licensee provided LPA with evidence that the violation was corrected in the time frame allotted.

This visit is being made to correct the previous report, and to retract the civil penalty that was issued. The citation has been cleared and a copy of the clearance letter provided.
