**** This is an Amended Document. LPA reviewed records and observed Licensee did in fact submit requested documents within the Plan of Correction timeline. The Civil penalty was not warranted.***** At approximately 9:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to conduct a Plan of Correction (POC) visit. Upon arrival, LPA observed only one staff present at the facility. On 07/18/2025, LPA issued a citation for a violation of California Code of Regulation section 87465(g) with a plan of correction stating Licensee will ensure timely medical attention is sought for residents. Licensee agrees to submit a plan and procedure regarding residents needing medical attention due to any condition appearing life threatening and how medical attention will be provided. Licensee will submit plan and procedure to LPA by POC date of 07/21/2025. As of today's date, the requested documents have not been received. A civil penalty is being issued in the amount of $100 per day for eight (8) days.

On 07/18/2025, LPA issued a citation for a violation of Health and Safety code section 1569.269(6), with a plan of correction stating Licensee will ensure Resident rights are maintained at all times. Licensee agrees to review and retrain all staff on Resident Rights and to ensure staffing levels are appropriate for the needs of residents in care. Licensee shall submit self certification that all staff have been retrained on Personal Rights and shall submit an updated LIC500, Personnel Summary to CCLD by 07/21/2025. As of today's date, the requested documents have not been received. A civil penalty is being issued in the amount of $100 per day for eight (8) days. The total amount of today's civil penalty is $1600. ***Civil Penalty was withdrawn***

Administrator Jean Rice had not arrived at the facility by the end of this visit.

Failure to correct the cited deficiency(ies), will result in further civil penalty assessments.

This report was reviewed with Lupo Canilao and Appeal rights were given. A copy of this report and LIC421 forms will be emailed to the Licensee.
