Licensing Program Analyst (LPA) Magdaleno arrived unannounced to follow up on a self-reported incident at this facility and was greeted by Alex Macajola, House Manager. Gabriela Echevarria, Administrator, was contacted via telephone and provided further information regarding incident.
During this visit LPA gathered documents and interviewed staff.
No deficiencies cited. Exit interview conducted with House Manager, whose signature on form confirms receipt.
