Facility Evaluation Report
Licensing Program Analyst (LPA) Cuadra conducted an unannounced Annual Required – 1 yr. inspection to this facility and met with Administrator, Fritzie Cube. Annual fees are current. Required postings were observed. Contact information was reviewed. Clients were attending to day program.
LPA/Administrator toured the inside and outside of the facility and made the following observations: facility was found to be clean and at a comfortable temperature with all exits free from obstruction. Client’s bedrooms are furnished per regulation. Fire Extinguisher was found to be last charged and serviced on 9/4/2024. Smoke Detectors & Carbon monoxide detectors were found to be operational during the visit. Last emergency/fire drill was conducted and documented on 8/8/25. There was a sufficient supply of both perishable and nonperishable foods as by Title 22 Regulations. Facility has plenty of Emergency food boxed and labeled in the garage. Toxins are stored in a locked closet in the laundry room & under kitchen sink. Medications are stored locked and inaccessible to clients in locked medication cart next to kitchen. There was a supply of hygiene products and paper products available for clients. Hot water temperature measured 105.3 and 105.6 degrees F which is within acceptable regulations of 105 to 120 degrees F.
LPA initiated file review at 10:45am and reviewed four clients and three staff files. All client files have medical assessments and care plans are updated. All staff records are have current required training hours and CPR/1st aid certificates. Administrator certificate for Fritzie Cube #6067806735 expires 11/12/2025. Cash resources were reviewed. During today's visit, LPA learned that there is new administrator, but the change was not effective in the system, so LPA obtained the following documentation: LIC 308 Designation of responsibility, LIC 500 Personnel Summary, LIC 200 (original), First Aid Certificate, LIC 501 Personnel Record, Copy of Personal ID, Copy of licensee's resolution meeting signed and copy of liability insurance.
No citations issued during today's visit. Exit interview was conducted with Administrator and copy was given.
