Facility Evaluation Report
06/19/2025, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. There are currently 3 clients in care. One client was present during inspection. Two clients were attending day program. LPA was greeted by staff member. Administrator, Alexis Patton arrived shortly after. Administrator had an appointment for a client at 12pm and had to leave to ensure client made it to their appointment, therefore left staff member to sign report on their behalf.
LPA and staff member toured the building and grounds. The facility was found to be at a comfortable temperature. LPA observed a 2 day supply of perishable and 7 day supply of non-perishable food.
All rooms were furnished per regulation. All rooms were in good repair. Extra hygiene products and linens were available in a hallway closet. Water temperature in 2 of 2 sinks accessible to clients in care were measured at 116.9 and 134.0 degrees F (Technical Violation) , per conversation with administrator they will have maintenance come out and turn down the water heater to ensure water temperature is within the regulation range of 105 to 120 degrees F. Fire extinguishers were last inspected 02/2024, per conversation with administrator they will get them serviced today (06/19/2024). Smoke/Carbon Monoxide detectors located throughout the facility were tested and LPA found one smoke detector located in bedroom #3 to not be operational at time of inspection (Technical Violation) . Toxins, sharps and other items that could pose threat if available to clients were located in the garage and in a hallway closet and found to be secured. Medications were found to be centrally stored in the hallway closet. LPA conducted spot medication count and found all prescription medication to be properly recorded on the Centrally Stored Medication Record. Facility conducts fire/disaster drills quarterly. Client cash resources could not be reviewed as they were not available at facility during inspection (Technical Violation) .
