Facility Evaluation Report

At approximately 2:40 PM Licensing Program Analyst (LPA) Magdaleno arrived unannounced to conduct a case management - other visit and was greeted by Caretaker Kenneth Gutierrez. Administrator Rose Salgado was contacted via telephone and arrived at approximately 3:05 PM.

The reason for this visit is to follow-up on an Incident Report (IR) submitted by facility to Community Care Licensing (CCL) on 12/22/2025. IR is regarding a potential eviction notice presented to Client (C1) as of November 2025. Interview with Administrator indicated that C1 still resides at the home and has not been evicted. Administrator stated that C1 was given a verbal and written eviction notice but has not been enforced. CCL has not approved eviction for C1. LPA informed Administrator of proper eviction procedures, Administrator will submit eviction notice to CCL for review.

No deficencies cited. Exit interview conducted with Administrator, whose signature on form confirms receipt.
