Facility Evaluation Report

An office meeting was conducted today in the Santa Rosa Regional Office with Licensing Program Manager (LPM), Victoria Bertozzi, Licensing Program Analyst (LPA), Christi Coppo, Licensee Millie Smith and Administrator, Cheryl Davidson regarding the passing of Licensee, John Smith. Reggie Smith and Jacqueline Brown Turner were also in attendance.

CCL requested the following documents to be brought for all three licensed homes (Jo-Mil Group Home, 480102381, Downie St. Home, 486803446 and The Gilcrest Group Home, 480111090):

· New financial documents in surviving spouse’s name

During today's meeting we received four of five documents and reviewed financial documents. Parties discussed potential Change of Ownership options under which a new license could be issued. Licensee will discuss their options and let CCL know once a decision is made as to the avenue they choose for potential future licensure.

No deficiencies cited during today's office meeting.
