Facility Evaluation Report

At approximately 2:00PM, Licensing Program Analyst (LPA) Magdaleno arrived unannounced for the purpose of conducting a Case Management - Other visit and met with Housemanager Tracy Skillman.

This visit was conducted to follow-up and gather more information regarding an Incident Reports (IR) submitted by the facility on 05/20/2026. IR indicated that a client (C1) had been caught shoplifting and due to an active warrant was detained by police followed by incarceration. LPA gathered documents, requested further documentation, made observations, and conducted interviews.

No deficiencies cited. Exit interview conducted with Housemanager, whose signature on form confirms receipt.
