Facility Evaluation Report
Licensing Program Analyst (LPA) Cuadra conducted an unannounced Annual Required – 1 yr. inspection for this facility and was greeted by Licensee,Eunice Perez. Clients were attending day program.
LPA/Licensee toured the facility inside and outside: It was found to be clean and in good repair at the time of the inspection with all exits free from obstruction. Clients rooms furnished per regulation. Fire Extinguishers were charged and serviced as of January 2025. Extra linens and hygiene products are available. Water temperature in client bathrooms was within regulation of 105 and 120 degrees F. Bathrooms were furnished per regulation with skid mats. All trash cans have tight fitting covers. At least two days of perishable and one week of non-perishable foods were available. Additional non-perishable food is stored in the kitchen pantry and at a closet by the hallway. Emergency food and water observed in the garage as well as a back up generator. Toxins were inspected and are stored and locked with magnet locks through the facility. Smoke detectors and carbon monoxide detector were tested and operational. Last disaster drill was conducted on July 27, 2025. Complete first aid kit, activity and menu was observed. LPA initiated file review at 1:00 pm of three staff files and three client's file was reviewed. Clients have medical assessment and care plan on file. Staff have required CPR/1st aid certificate and additional training 20 hours complete. Administrator certificate for administrator Ronaldo Perez #7034003735 expires on 3/16/2026. Medication and medication records reviewed. Cash resources were reviewed.
Licensee agreed to submit updates of the following by 10/17/2025: LIC500- Personnel Report, LIC308- Designation of Responsibility, liability insurance and surety bond.
No deficiencies were cited during this inspection. Exit interview was conducted with Licensee, and a copy of this report was given.
