Facility Evaluation Report

07/09/2025, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. There are currently 0 clients in care. Licensee stated they don't expect to have clients for approximately for another 6 months. Licensee is currently working with GGRC to receive clients.

LPA and Licensee toured the building and grounds. The facility was found to be at a comfortable temperature. LPA observed a 2 day supply of perishable and 7 day supply of non-perishable food. All rooms were furnished per regulation. Water temperature in sinks accessible to clients were measured at 109.1 degrees F within the range of 105 to 120 degrees F. Fire extinguishers were found to be fully charged. Smoke/carbon monoxide detectors located throughout the facility were tested and operational. The backyard area was found to be clean and exits were found to be obstruction free. Administrator Certificate 605847735 for Jose Rios expired 06/26/2025, per conversation with Licensee they will be scheduling an appointment to recertify their administrator certificate. LPA reviewed two staff records.

No deficiencies cited during today's inspection. Updated copies of the following documents were requested for facility file and are to be submitted to CCL by 08/09/2025:
