Facility Evaluation Report
05/18/2026, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. Facility is an Adult Day Care Program. LPA met with Executive Director, Stacey Martinez, Service Director, Marianita Vieira, and Day Program Manager, Tamara Huizen. Day Program has a fire clearance for 30 clients.
LPA and Executive Director toured the building and grounds. The facility was found to be at a comfortable temperature. There are 25 clients that are enrolled in day program. Today there are 22 clients in attendance.
Day Program has multiple activity rooms. LPA observed clients participating in activities during the tour. Facility has activities available to clients such as puzzles, games, and art supplies. Water temperature in sinks accessible to clients were measured at 118.3 and 117.1 degrees F which is within the range of 105 to 120 degrees F. Facility has five fire extinguishers and all were found to be last inspected 07/2025. Facilities last fire drill was conducted 03/2026. Facility has a third party company that tests and inspects their alarm system which was last inspected 03/24/2026. Sharps, toxins, and chemicals were locked and secured in the hallway closet. Facility does not handle medications or client cash resources.
LPA conducted a review of 5 client records. All records had the required documentation, however client (C1) did not have a physicians report on file. (Technical Assistance)
LPA conducted review of 5 staff records/training. Upon a review of staff records, LPA found all staff to have required annual and initial training as well as current 1st Aid & CPR certification on file.
