Facility Evaluation Report
On 05/22/2025 Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr inspection for this facility and met with Day Program Manager and Service Director. The facility currently provides care for 26 clients, 21 of which were at the facility at the time of visit. Day Program operates Monday through Friday.
LPA conducted tour with Day Program Manager; facility was found to be clean and at a comfortable temperature with all exits free from obstruction. Fire extinguishers were found to be last charged 07/2024. Maintenance logs are documented on file. Last emergency disaster drill was conducted 03/14/2025. Toxins, cleaning supplies and other potentially dangerous items are to be stored in a designated cleaning supply closet inaccessible to clients.
Clients were observed to be engaged in group activities and other options to choose during program hours. Facility designates several Direct Support Staff to accompany clients by group. Day Program has a system that allows clients to pick which activities they would prefer from most favorite to least, they then are selected by their picks for the following month to ensure clients are participating in activities they prefer.
The facility does not handle either client cash resources or medications within the program. LPA conducted a file review of client records and found all records to be in order. In addition, LPA conducted a file review of staff records and found staff to have required annual training and CPR & 1st Aid training completed.
