Facility Evaluation Report

A Non-Compliance Conference was conducted today in the Santa Rosa Regional Office. Present in the meeting were, Regional Manager, Carla Nuti-Martinez, Licensing Program Manager (LPM) Victoria Bertozzi, Licensing Program Analysts (LPAs) Caitlynn Felias and Ali Deniz, Executive Director, Camille Brown, and Regional Director of Operations, Karen Enciso. The purpose of today’s Non-Compliance (NCC) meeting was to address areas of concern identified by the Department.

· Staff not adequately supervising residents in care

· Lack of supervision resulting in resident injuries

· Changes to LLC/Management Company without notifying the Department
