Facility Evaluation Report
On 01/14/2026, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct an investigation and met with facility administrator, Camille Brown.
Community Care Licensing (CCL) conducted an investigation regarding an allegation of sexual abuse. The facility reported that staff member S1 sexually abused a resident while providing incontinence care. The facility reported the allegation to local law enforcement, Community Care Licensing, and the Long-Term Care Ombudsman, as required of mandated reporters. Staff member S1 was arrested and later pled guilty to a misdemeanor charge of assault and battery.
No deficiencies were cited as a result of this investigation.
