Complaint Investigation Report

The following determination were made based on information gathered: Interviews with Administrator and staff, and records reviewed indicate facility reported based on tittle 22 requirements. Therefore, based upon information gathered there were no violation to regulation 87211, Reporting Requirements and the Complaint is UNSUBSTANTIATED.

A finding that the complaint allegation Facility failed to report incident of staff bringing a weapon on property is found to be unsubstantiated meaning that although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is UNSUBSTANTIATED.

No deficiencies cited during today's visit.
