Facility Evaluation Report

Licensing Program Analyst (LPA) Contreras arrived unannounced to conduct a required annual inspection visit. Upon arrival, LPA rang doorbell and called facility phone but received no response. LPA attempted second phone and did not receive response. Upon fourth attempt to call facility, LPA received answer from admin.

Admin told LPA that there was no one available to come to facility and all clients were currently at day program. LPA let admin know to have someone available but admin was unable to come to facility. LPA also let admin know to update facility phone number as number listed in facility file was not operational.

LPA will attempt to come at another time.
