Facility Evaluation Report
Licensing Program Analyst (LPA), Shannan Hansen arrived unannounced at facility, for the purpose of following-up on a self-reported incident report facility submitted to Community Care Licensing (CCL) on 3/10/2025. LPA was greeted by staff Dayzunique who contacted Administrator, Garrett Johnson that arrived shortly after. There is a total of 4 clients in care, 2 were at programs at time of visit.
CCL received an incident report reporting on the evening of 3/7/2025 while staff were checking/cleaning C1’s room, as part of clients’ regular room care support observed several medication doses in ziplock bag, which were removed.
Previously, CCL received an incident report (regarding same client) on 12/09/2024 that on 12/4/2024 per report, facility found past cycle medications for a client in their room. Medications were provided to client to take when on home visits or at work. Report states that client is own responsible party and allowed to administer own medications. LPA obtained documents dated 1/15/2025 indicating this is true.
Facility/NBRC/SC/Turning Point had meeting with client to discuss danger/harm that could result from non compliance of medication.
