Facility Evaluation Report

Licensing Program Analysts Willis and Felias arrived unannounced to conduct a case management inspection and met with Administrator Morgan Ray.

LPAs spoke with Administrator regarding a recent self-reported incident regarding allegations of sexual and physical abuse. Facility cross-reported to the local law enforcement who conducted an investigation. Based on conversation with Administrator and review of police report, allegations of assault could not be confirmed. Facility has increased supervision as a precaution and have discussed consent with clients.

No deficiencies cited during this inspection.
