COMPLAINT CONTROL NUMBER: 21-AS-20240918141157
Neglect/lack of supervision resulting in client wandering from facility.
Licensing Program Analyst (LPA) Shannan Hansen arrived unannounced to initiate a complaint investigation and subsequently delivered findings regarding the allegation listed above. LPA met with Administrator Amanda Simmons.
Neglect/lack of supervision resulting in client wandering from facility- Complainant alleges client was in neighbor’s backyard unsupervised and going through garbage can. LPA confirmed through interviews and pictures. Client was in neighbor’s backyard without staff supervision digging through garbage can.
A finding that the complaint is Substantiated means that the allegation is valid because the preponderance of the evidence standard has been met.
The following deficiencies were observed (see LIC 9099D) and cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. Exit interview conducted and appeal of rights provided.
