Facility Evaluation Report

On 07/02/2025 at approximately 09:35AM, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct 1-Year Required visit of this licensed Adult Residential Facility (ARF). LPA was greeted by Administrator, Amanda Simmons. The facility currently provides care for 4 clients, some of which were at program or preparing for outings.Facility has an approved fire clearance for 2 ambulatory and 2 non-ambulatory residents. Upon arrival, LPA was informed that there were 3 staff members on-site. Facility is a 1 story building with 4 Resident bedrooms, 2 bathrooms, and common spaces.

At approximately 10:15AM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation.

At approximately 10:45 AM, the LPA and Administrator conducted a tour of the building and grounds, which were observed to be clean and in good repair. However, Resident Room #3 showed damage to the walls, reportedly due to a client’s aggressive behavior. The Administrator stated that the electrical outlet on the wall would be repaired later today. Resident Room #4 was filled with an excessive amount of personal items, occupying most of the available space and leaving limited room for movement. The cluttered and unorganized condition of the room poses a potential health and safety concern for the client in care (Technical Violation Given).

Bathroom number #2 had water leaking outside of the shower area. The shower floor appears to be improperly sealed, causing water to flow out onto the bathroom floor. Additionally, sections of the tile were found to be rotting, indicating possible long-term water damage. (Technical Violation Given). Administrator agreed to submit plan of correction statement along with pictures before 07/23/2025.
