This is an official report of an unannounced visit/investigation of a complaint received in our office on

09/02/2022 and conducted by Evaluator Christopher Arnhold

COMPLAINT CONTROL NUMBER: 21-AS-20220902134559

At approximately 8:30AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct an investigation into the above allegation. LPA met with Licensed Phyciatric Technician (LPT) Amanda Simmons, and reviewed records. Based on a review of records and interviews conducted, there is not evidence to support the allegation that staff caused an injury to C1. LPA reviewed documentation that shows C1 being aggressive to staff as well as documentation of bruises on their legs. Based on interviews conducted, C1 routinely bumps into a dresser in their room and the bed frame, causing bruising to thighs and shins. Staff observed redness under the eyes and cheeks after C1 returned from day program. Based on interviews conducted, C1 is prone to having reactions from various foods. The facility has brought this to the attention of the physician for a solution. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is Unsubstantiated.
