Facility Evaluation Report

An office meeting was conducted today, 02/25/2026, in the Santa Rosa Regional Office. The following individuals were present in the meeting: Regional Manager, Carla Martinez, Licensing Program Manager, Kim Mota, Licensing Program Analyst, Ethel Contreras, Facility Program Administrator/Licensee Erika Diaz, and Facility Administrator/Licensee Aldrin Eliseo. The purpose of the informal office meeting was to discuss facility Care and Supervision and Program Plan.

Items addressed in today's meeting include but are not limited to the areas below :

Licensee's will re-evaluate program plan and decide to submit exceptions for client in care.

Follow up on client's C1 LIC602, if they are able to leave unassisted.

No deficiencies were cited during today's office meeting.
