Facility Evaluation Report
Licensing Program Analyst (LPA), Farhaan Sarangi arrived unannounced at Paramount House Senior Living for the purpose of conducting a Case Management-Incident inspection regarding an SOC 341 that was forwarded to the Santa Rosa Regional Office. LPA was greeted at the door by Administrator, Richard Remigio and was granted access into the facility.
During the Case Management-Incident inspection, LPA interviewed staff and residents. In addition, LPA requested the following documents:
No deficiencies were observed or cited during this Case Management-Incident Inspection. Exit interview was conducted and a copy of this report was given to the Administrator.
