Facility Evaluation Report
Licensing Program Analyst (LPA) Contreras conducted an unannounced case management and met with Administrator( Admin) Jacqueline Hicks. The purpose of the case management visit was to obtain additional information regarding two recent incident reports and SOC 341 that was self-reported by the facility on 8/19/2025 and 8/22/2025.
LPA received unusual incident report and SOC341 involving client (C1) alleged physical abuse dated 8/17/2025. Per incident report, on 8/17/25 while staff was assisting C1 with activity of daily living observed a large bruise on C1’s left shoulder, when asked about the cause of the bruise, C1 did not respond and continued getting dressed, but did not report any pain and stated staff’s name (S1). Due to the allegation of physical abuse law enforcement was notified and registry staff is hired through.
LPA received second incident reported on 8/22/2025 and SOC341 involving client (C2) alleged physical abuse, per incident report, on 8/19/25 C2 stated they got scratched by staff and stated staff yelled at them. C2 stated it was done on purpose and stated staffs name (S2). Due to allegation of abuse law enforcement was notified and staff has been placed on administrative leave.
During today’s visit, LPA requested and reviewed records, made observations and conducted interviews with staff and clients in care. The Department will review documentation obtained to determine if further actions are needed.
No deficiencies cited during today’s inspection.
