Complaint Investigation Report
Per interview, bus driver informs staff when C1 will be getting dropped off but did not let staff know on 3/12/2026, leading to C1's missing person report. Per schedule review, seven staff were on duty at facility during time C1 went missing and 7 out of 7 staff were not informed of the drop off. Based on four staff interviews, all confirmed that C1 did not go inside home after van driver dropped C1 off. Based on camera footage, staff observed C1 stood outside the front door and once bus drove off, C1 left front door and walked away alone. C1 did not knock or enter house code. In addition, camera footage reviewed by admin and staff, it was observed that bus driver drove away before C1 could get inside the facility.
So, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
