The inspector’s account

Licensing Program Analyst arrived unannounced to conduct a Case Management- Deficiency Visit and was greeted by administrator (admin) Stephen Ngata. Case management is being conducted to address deficiencies that were identified during complaint findings delivered today, (see LIC9099).

On 3/12/2026 Vacaville PD was called by S1 to initiate a missing person report regarding C1. C1 was then found five miles away from facility. Facility failed to report incident to CCLD within the Reporting Requirements Regulation under Title 22. Based on CCLD Reporting Requirements, a written report shall be submitted within seven days following the occurrence of an event. Events reported include any unusual incident or client absence which threatens the physical or emotional health or safety of any client, see LIC809D. Licensee did not submit written report or let CCLD know about incident that occurred involving client.

LPA had conversation with admin regarding reporting requirements. LPA printed Reporting Requirements and admin to read and review regulation.

Deficiencies are cited from the California Code of Regulations, Title 22, Division 6 of California Regulation and the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, or repeat violations within a 12-month period, may result in a civil penalty assessment. Appeal rights were provided.
