Facility Evaluation Report

Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced to complete a case management investigation. LPA met with Yolanda Harrell, Administrator, regarding an incident which was self-reported on 02/23/2023 between resident R1 and resident R2, occurring on 02/17/2023.

The facility provided documentation showing that residents had been re-assessed, responsible parties notified and care plans adjusted in order to support the changing needs of residents in care.

There were no deficiencies found at the time of inspection.
