Facility Evaluation Report
On 12/09/2025, Licensing Program Analyst (LPA) Jill Nakagawa, arrived at the facility unannounced to conduct a 1-Year Required Annual Inspection. LPA met with Facility Administrator Brenda Slay and explained the purpose of the visit.
LPA and Administrator toured facility together to ensure health and safety of residents in care. The facility was decorated for the holidays, which made the facility warm and homey. Areas toured included but are not limited to: common areas, resident bedrooms, restrooms. LPA observed the facility to be clean, in good repair and odor-free. Each bathroom has the necessary grab bars, non-skid flooring or shower chair, paper towels. LPA observed each bedroom to have the necessary furnishings with working lights and windows with screens and resident's rooms showed their preferences and personalities.
Facility has a 2-day perishable and a 7-day non-perishable amount of food. Sharps and medications are locked and inaccessible to clients. Hot water temperature was measured within the required range. LPA observed fire extinguisher was last serviced on 11/11/2025 . A fire drill was conducted on 10/01/2025. Carbon monoxide detector was tested and found operational. There is a central fire alarm system throughout the facility. LPA observed multiple first aid kits available for use both upstairs and downstairs. An evacuation chair is available on the upstairs landing.
In the areas toured no immediate health, safety, or personal rights violations were observed.
LPA reviewed a total of two (2) residents files and one(1) new staff file which contained all the required documentation.
No deficiencies are being cited as a result of today’s inspection.
