Facility Evaluation Report

Licensing Program Analyst (LPA) Walters arrived unannounced to conduct a REQUIRED 1 Year Annual Inspection. LPA was greeted by staff. Administrator, Vanessa Baldwin arrived later. This visit is focused on the infection control of this facility. The facility submitted an infection control mitigation plan to Community Care Licensing, which was approved on 07/28/21. At the time of inspection, there were two staff providing care and supervision to two clients. Two additional staff arrived later.

The facilities visitor policy was posted on the exterior door. Staff conducted a screening and temperature check of visitor upon entry. All visitors are signed in using a sign in sheet. There were Personal Protective Equipment (PPE) on a entry side table, including: disposable mask, N-95, hand sanitizer and gowns. Temperature's and vaccine information of staff, clients and visitors, are logged and stored within facility binders. Clients are monitored for symptoms throughout the day. Each client has the ability to isolate independently in the event that there is an outbreak.

Signs were posted throughout the facility to promote physical distancing and droplet precaution. Facility staff utilize a daily cleaning chart to clean high touch and common areas. Per Administrator, the facility is disinfected during every shift. LPA observed a 30 day supply of PPE, incontinence products and medications. Bathrooms were stocked with hand washing supplies. LPA and Administrator discussed replacing cloth towels with paper towels, and having trash cans with lids, to prevent the spread of infectious and airborne illnesses. PIN-21-53 regarding Vaccination And Booster Requirements was also discussed, and copy was provided to Administrator.

No deficiencies were cited during today's visit. Exit interview conducted with Administrator, Vanessa Baldwin.
