Complaint Investigation Report

The complaint alleges that Facility staff hit Client C1 and Facility staff spoke inappropriately to C1. LPA conducted interviews and made observations LPA could find no evidence to corroborate the allegations. Through interviews, it was learned that C1 made the above allegations, but later recanted them. LPA was unable to find any evidence to corroborate the above allegations.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegations are unsubstantiated.
