Facility Evaluation Report

LIcensing Program Analyst (LPA) Jill Nakagawa conducted an unannounced case management and met with Program Director Jeramie Nowlin. The purpose of this case management inspection is to follow up on an Immediate Exclusion from the facility for Staff (S1).

LPA provided a copy of the letter "Order to Licensee/Facility of Immediate Exclusion from Facility" dated May 13, 2025 for S1. Program Director told LPA S1 was currently on staff for Dungarvin's community-based program, which is not licensed through Community Care Licensing. .

Based on evidence obtained during today's visit, the LPA has verified the individual is not present, working at or residing at the facility #486801931.LPA requested that S1 be disassociated from their Guardian Roster.

No deficiencies cited during today's inspection. Copy of letter provided to Jeramie Nowlin, Program Director.
