Facility Evaluation Report

Licensing Program Analyst (LPA) Contreras arrived announced at approximately 2:50PM to conduct an inspection in regards to the voluntary closure of this facility. LPA met with Administrator (admin) Sara Trail, and Facility Director Cedric Tanner also present.

LPA toured the facility and observed no sign of clients residing at the facility. All client medications have been removed. Furniture is still in rooms but personal belongings have been removed. All clients have been relocated to their new residences without incident. LPA was advised that last client moved out was on 4/20/2026. Two client rooms are now being used for staff.

LPA will send out forfeiture letter to Licensee to complete the closure process.

This report was amended to correct the day clients moved out on 4/20/2025. LPA returned facility license to Facility Director Cedric Tanner.
