Facility Evaluation Report

An informal conference was conducted today in the Santa Rosa Regional Office. Present in the meeting were Santa Rosa Regional Manager Carla Martinez, Licensing Program Manager Kimberley Mota - Licensee, Sara Trail, Facility Director, Cedric Tanner - North Bay Regional Center representatives Katy Vanzant and Isabel Tronchoso.

The purpose of the informal office meeting was to discuss areas of concerns regarding change of use of facility.

Issues discussed during the meeting were:

Client (C2) staffing will remain in place 24/7 until the change of use of facility

SLS model and CCL requirements while facility is transitioning.

C1 transitioned to new facility on 5/15/26. Licensee ensured that staffing will remain in place for C2. When facility does change their model to SLS, staffing will remain 24/7 or as mandated.

No deficiencies were cited during the office visit.
