Facility Evaluation Report
Licensing Program Analyst Contreras (LPA) and Licensing Program Manager (LPM) Kim Mota arrived unannounced to conduct a case management visit and was greeted by staff Ibou Diop . Facility Director Cedric Tanner arrived shortly after. The purpose of the visit was to follow up regarding closure of the facility. LPA and LPM made observations and gathered documents.
Upon arrival, staff (S1) not associated to facility roster (Deficiency Cited, see 809D). It was previously reported that clients (C1 & C2) had moved out as of 4/20/2026. In further discussions with Licensee, it was determined that C1 & C2 had both relocated to family residence but then returned to live at facility. C1 returned to facility on 4/29/2026 and C2 returned to facility 4/30/2026. During a tour of the facility, LPA observed staff now live in C1 former room and C2 is in their previous room. On previous inspection of 4/28/2026, both of these rooms were locked and Licensee informed LPA that those rooms belonged to staff S2 and S3.
Due to clients remaining in the home until relocation and or facility is licensed under a different model outside of Title 22 Regulations, LPA returned facility license to display and informed Facility Director that the facility is currently still licensed by CCL and must adhere to Title 22 Regulations.
Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation and the Health and Safety Code. Appeal rights given and discussed with Administrator. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties.
