Facility Evaluation Report
Licensing Program Analyst (LPA) Contreras arrived unannounced to conduct a required annual inspection and was greeted by staff and floor supervisor Marcela Eduardo. Administrator Samika Simmons unable to attend. Caiitlin Velos, administrative assistant arrived shortly after. Facility is a day program with an approved fire clearance for 20 non-ambulatory, not to exceed a total capacity of 45. Day program hours are from 7:00am to 4:00pm.
LPA toured the building and grounds which was observed to be at a clean and comfortable temperature. There were 11 participants present at facility during visit. Some participants were on an outings either at the mall or food bank. Participants bring their own lunch except some Thursdays when program has cooking classes. Water temperature in sink accessible to participants in care measured at 119 degrees F which is found within the allowable range of 105 to 120 degrees Fire extinguishers were last inspected 10/02/2025. Monoxide detectors located throughout the facility were tested and operational. Fire and smoke alarms are wired through the facility building by the fire department. Facility unable to provide proof of disaster drills (Deficiency Cited, see 809D).
Facility has 4 vans. 3 vans were inspected. 3 out of 3 vans inspected were observed to have a fire extinguisher. One van observed to not have a first aid kit and second van observed to not have a complete first aid kit . LPA had conversation with admin to ensure that all first aid kits are fully stocked and included in all vans. LPA observed participants and staff to be engaging in activities including coloring and reading cards.
