The inspector’s account

At approximately 10:00am, Licensing Program Analyst (LPA) Contreras arrived unannounced to conduct a required annual inspection and was greeted by staff Charles Anderson. Assistant Administrator Ashley Fernandez and Administrator Francis Fernandez arrived shortly after. Facility is an Adult Residential Enhanced Behavioral Support Home with an approved capacity for 4 ambulatory clients and a delayed egress. There are currently 3 clients in care.

LPA toured the building and grounds which was found to be clean and in good repair. LPA observed all walkways and exits to be unobstructed inside facility. Facility does not have Let Us Know Complaint and See Something Say Something (PUB475) poster in visible area. LPA had conversation that poster must be up at all times and admin placed PUB 475 during visit (Technical Advisory given).

Clients have access to outdoor use with outdoor exits and walkways unobstructed. Facility has large shed with three smaller sheds in back of property. All storage sheds were unlocked by admin and LPA observed all sheds being used for storage. All bedrooms found in good repair. All bedrooms were observed to have clean linens, night stand, chair and light. LPA observed at least a 2- day supply of perishable and 7-day supply of non-perishable food. Facility kitchen, refrigerators and freezers were clean. Snacks accessible to clients. Expiration dates noted in food and toxins are inaccessible to clients. Sharps and knives were locked and inaccessible to clients . Emergency food and water observed with first aid kit.

Bathroom accessible to clients had bath bats and paper towels. Water temperature measured within regulation at faucets accessible to clients. Fire extinguishers were charged and inspected on 01/2024. Carbon and fire alarms were tested and operational. Disaster Drills are recorded and conducted quarterly, last drill conducted on 11/10/2025.
