On 09/05/2025 at approximately 09:35AM, Licensing Program Analyst (LPA) Ali Deniz made an unannounced Post Licensing inspection of this senior care facility.
Upon arrival, there no one was at the facility. LPA returned to the car and placed a call in attempt to have someone meet LPA at the facility for inspection. No one responded after repeated calls and knocking. LPA left a voicemail at (408) 382-1930 .
A follow-up visit will be attempted in the next few days.
