Facility Evaluation Report
Licensing Program Analyst (LPA) Cuadra arrived unannounced to conduct a Required-1 Year visit and met with Administrator, Maria Sickmen. Annual fees are current. Contact information was reviewed.
LPA/Administrator toured the facility inside and outside, which was found to be clean and a comfortable temperature of 70 degrees. LPA observed all walkways and exits to be unobstructed. The amount of fresh and non-perishable foods was within regulation. Toxins are secured in a locked cabinet in the garage and under kitchen sink and not accessible to clients. There is a sufficient supply of hygiene products and linens on hand for client use. Water temperature measured at 117 degrees F, which is within regulation between 105 and 120 degrees F. Fire extinguishers were charged and service on July 2025. Smoke detectors and carbon monoxide were tested and found to be operational. Last disaster drill conducted 5/3/25. Medication is centrally stored in a locked cabinet in the living room. Required postings were observed.
At 10:00am LPA initiated file review of four client files and three staff files. All client's files have current care plans and medical assessments. All staff have required CPR/1st aid certificates and required training hours complete. Cash resources and records were reviewed. Medication and medication records were reviewed. Maria Sickmen Administrator Certificate 7029085735 expires 11/07/2026.
Administrator submitted updates of the following: Personnel Report (LIC500), Designation of facility Responsibility (LIC308), Affidavit Regarding Client Cash Resources (LIC400) , Copy of Surety Bond and liability insurance.
No deficiencies were found during the inspection. Exit interview conducted with Administrator and a copy of this report was given.
