Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced to conduct a Required 1-year visit of the facility. LPA was welcomed by David Price, Licensee. There were no residents at the facility as the facility is currently undergoing renovations..

The facility will continue to operate when renovations are completed. Licensee will inform Dept. when the facility is ready for inspection
