Facility Evaluation Report

Licensing Program Analyst Jill Nakagawa arrived unannounced on 03/24/2025 at approximately 2:30 PM to perform an annual inspection. The facility currently has no clients as it is going through renovations. All clients were relocated to another facility within the organization.

LPA arrived at the facility and was allowed access. Licensee Sally Price and LPA toured the facility and found no one on the premises.

The perimeter of the facility has been fenced. Additional improvements to the property are being completed including new walkways on the outside of the house and modernization to the interior.

Licensee will inform CCL when the facility has completed the work and ready for clients to return.
