Facility Evaluation Report

Licensing Program Analyst (LPA) Jill Nakagawa conducted an unannounced Annual Required – 1 yr. inspection to this facility and met with Operations Specialist Adriana Price and newly appointed Administrator Sabrina Clark. There are currently 3 clients at the facility; all were participating in Day Programs during LPA's visit. DSP was on site as well.

During facility tour on 04/28/2026 facility was found to be organized, free of clutter with all exits free from obstruction. Clients' bedrooms, common areas, kitchen & food storage areas were inspected. Two (2) Fire Extinguishers were found to be fully charged and last inspected on 07/16/2025. There were 2 carbon monoxide detectors that were tested and functional. Smoke detectors were checked; batteries last replaced on 03/26/2026. Toxins are stored in a locked cabinet inside the staff room. Dangerous items were stored inaccessible to clients. There was a supply of cleaners, hygiene products and paper products available for clients. All clients' bedrooms have lighting and appropriate furnishings. LPA inspected the back yard, which was free of debris. There is a patio with cover providing seating with shade. There was a locked shed in the back for storage of generator and other household goods.

The living room was set up for clients to enjoy TV and games, or visit with friends and family. The staff room was well-organized. Four (4) staff and three (3) client files were inspected and found to be up-to-date, well-organized and complete.

LPA and new Administrator went over the Infection Control Plan and the Emergency Disaster Plan, and reviewed the medication management training.

There were no deficiencies found at the time of inspection. No citations issued.
