Facility Evaluation Report
On 6/6/2025 at approximately 1:45 PM Licensing Program Analyst (LPA) Nakagawa arrived unannounced to conduct a 1-Year Annual Inspection. LPA was greeted by staff member Eileen, who allowed LPA entry and contacted Adminstrator, who was off site. There was one client and one staff member member on site at the time of visit. The other clients were participating in Day Program. Administrator Cora June Price arrived shortly.
The LPA found the facility to be clean and well-kept. The refrigerator was clean, with adequate perishable and non-perishable food as required, per Title 22. The fire clearance has been approved for 4 adults.
There were no accessible bodies of water or firearms. There is a non-operating, unfilled hot tub on site, with a secure cover. There was one (1) carbon monoxide detector which was tested and operational and (4) smoke detectors that were tested and operational An emergency disaster plan has been updated and last fire drill for all staff shifts was 04/24/2025.
LPA observed locked cabinets for sharps and medications. Bathrooms had slip mats and grab-bars for resident safety. Hand washing supplies and paper products were available in sinks used by residents. Water temperature was between 105-120 degrees F, which is within regulation. A tour of residents' bedrooms was conducted and bedrooms inspected have lighting & appropriate furnishings and linens.
LPA observed the following postings: personal rights, emergency disaster plan, activity poster and menu and the Let Us Know Complaint poster.
